From the four options listed below, I prefer that you use the On-line format as it is the easiest to fill in and quickest to submit. Alternatively, you may fill in the Word or Pages format and return as an email attachment or via fax. If you do not have Word or Pages, then print out the Adobe PDF version, fill it in neatly and legibly by hand (print in BLOCK capitals to make it easier for the franchisor to review) and then scan/email the form or fax it back to me at 603-888-6081.

Remember that ultimately I will be presenting you and your completed profile, with your permission of course, to the franchisor and so this form will be representing you. We want to make a great first impression, so please fill in the profile as completely as possible and if filling in a printed form by hand, be sure that it is neat and easy to read.

Due to the many inquiries we receive, a submitted Profile Form that contains just basic Client information with no Professional Experience/Education/Financial data will be rejected by the Franchisor. So please take the time to fill in the form as completely as you can.

If you have not already done so, you should also take the Core Values Index (CVI) Assessment. The CVI really helps our clients figure out their core values, who they are, what they are good at, primary/secondary operating modes etc. It will only take a few minutes to complete.
